At the top right of the page, across from "About trickortreat2014" there should be a button that says something like "Nominate"? (Mine says "My Nominations" now, so I'm not sure.) Anyway it'll take you to http://archiveofourown.org/tag_sets/477/nominations/new, where you can put in up to four fandoms with up to four characters for each.
If the fandom/character you're nominating already exists, you can pick it from the autocomplete; if not, you can just type it in. You don't have to do anything different for TV or movies or whatever--just put in the title and the tag is linked to the right medium. If the tag doesn't already exist, the wranglers will take care of it.
If you want to nominate a specific version of a canon where the title isn't specific enough, you can put something in the title to disambiguate it like "Ghost Soup (TV 1991)".
